Under Leadership of the Manager of Enabling Patient Access & Patient Flow, the Physiotherapist will work as a collaborative team member by supporting rehab patients (ALC in acute care), working on priority referrals, and supporting complex discharges from Trillium Health Centre to the community.
Key Qualifications:
- Must hold a current Independent Practice Certificate with the College of Physiotherapists of Ontario
- Candidate will have relevant experience in managing acute patients in a variety of programs: orthopaedic, neurological, medical, surgical and/or rehab adult populations
- Eligibility for active membership in the Canadian Physiotherapy Association
- Current certification in Basic Cardiac Life Support preferred
- Excellent assessment and treatment skills
- Leadership capacity within an interdisciplinary team
- Committed to the development of best practice intervention/therapy in a Complex Care/Transitional Care setting
- Ability to work with minimal direction/supervision
- A good understanding of and experience with the PTA role required
- Ability to work with PTA
- Good organizational, planning and time management skills
- Excellent communication skills
- Flexibility and adaptability to change are essential
- Computer skills for e-mail
Position Summary:
The Physiotherapist - Integrated Transition Team is responsible for providing assessment and treatment programs for referred patients, including patient and family education. The ability to act in a consultant role is important to effective management of the patient population. Assigns activities, as appropriate, to the assistant and supervises effectively.
