Position Summary:
The Quality & Patient Safety (QPS) Advisor serves as a key enabler in the development of a culture of safety and performance excellence at Trillium Health Centre. As a member of the Quality & Patient Safety (QPS) Team, you will collaborate with your QPS colleagues, front-line and system-level teams to achieve the desired outcomes in Trillium"s key strategic priority areas.
This will be accomplished by:
- Serving as an internal facilitator and coach to Directors, Managers and staff to assist with the planning, coordination and evaluation of quality improvement initiatives and patient safety practices.
- Establishing links with other leaders of quality improvement and patient safety processes across the organization including Decision Support, Risk Management, Professional Practice, and Patient Relations
- Identifying relevant quality and patient safety related training needs and delivering the training
- Collaborating with stakeholders to create and update policies and procedures in accordance with best practice and patient safety improvements
- Collaborating with health system leaders to plan, organize and direct activities to identify, evaluate, and take corrective action against identified safety issues/concerns that may result in injury to patients, visitors and employees and in property loss or damage or legal liability
- Utilizing the incident reporting system to promote patient safety and identify areas for improvement
- Managing critical incident reviews and investigations according to established frameworks
- Maintaining departmental protocols specific to patient safety
- Being knowledgeable about relevant legislation, regulations and their applications
- Providing expertise to clinical teams in the review of patient safety events through the use of various methods of analysis (Root Cause, Healthcare FMEA, Human Factors, Cause and Effect, etc.)
- Providing integration on safety-related initiatives such as Accreditation Canada ROP"s, Safer Healthcare Now! and other related activities
- Educating staff regarding new statutes and/or guidelines (e.g. Excellent Care for All Act), safety and quality improvement activities (e.g. Safer Healthcare Now).
- Supporting the development of performance indicators and measures to monitor organizational, team and/or service performance and ensure alignment with strategic direction.
- Providing regular reports on safety-related activities and improvements
- Analyzing and trending quality and patient safety data to determine potential areas for improvement
- Other duties as required
Key Qualifications:
- Master"s degree in a healthcare related field preferred. A Bachelor"s degree with appropriate prerequisite knowledge, experience and skills is essential.
- Sound knowledge and application of relevant legislation including the Excellent Care for All Act
- Demonstrated knowledge and experience with performance measurement, data reporting and analysis, quality improvement methodologies including Lean, and Root Cause Analysis
- Working knowledge of electronic incident management systems (e.g. RiskPro)
- Excellent interpersonal skills and demonstrated ability to relate well internally with all levels of staff, physicians, volunteers and external agencies
- Superior communication skills, both oral and written
- Demonstrated proficiency using Microsoft Office, including Word, Excel, PowerPoint, Project and Visio
- Proven ability to work independently with minimal supervision, take initiative and perform well in stressful environments
- Integrity in dealing with confidential matters; ability to handle difficult situations with tact, diplomacy and sensitivity
